To define hydrocele, it can be defined as excessive fluid accumulation between the membranes surrounding the testicle. If the abdominal membrane does not close completely and a small gap remains, a connection is formed that allows the fluid in the abdomen to enter the sac. This is called communicating hydrocele and is seen in childhood. The swelling sometimes decreases and sometimes increases. If the entrance part of the pouched abdominal membrane is wide and the intra-abdominal organs enter from here, a condition called inguinal hernia occurs and must be corrected with surgical treatment.
How Does Hydrocele Occur?
There are three types of hydrocele in men;
If the fluid is present both around the testicle and in the inguinal canal, and if the swelling decreases and increases with position, it is called a communicating hydrocele and is seen in childhood.
If the fluid in the membranes surrounding the testicle has no contact with the abdominal cavity and the fluid is limited only to the testicle area in the bag, it is a testicular hydrocele. Hydrocele occurring after varicocele surgeries, infection, testicular tumor, testicular trauma or unknown reasons falls into the group of testicular hydrocele.
Obstruction within or just outside the canal is defined as cord hydrocele.
How is the Treatment Process of Hydrocele?
The diagnosis of hydrocele is usually made during routine child checks during the neonatal period. The definitive and most efficient treatment of hydrocele is surgical intervention. However, since hydrocele can sometimes resolve on its own, surgery is not recommended in the first 12 months. The operation can only be performed earlier if a separate partology in the testicle is suspected or if there is an inguinal hernia. During surgery, the fluid in the hydrocele sac is drained and then the excess tissues in the sac are removed.
There is no specific time period for the treatment of the disease, which is generally seen in adults and defined as secondary hydrocele. It can be operated on at any time from the moment of diagnosis.
Hydrocele surgeries can be performed on a daily basis if there are no special circumstances. In day surgeries, there is no need for the patient to spend the night in the hospital. Routine preoperative observations of the patient are made one day before the surgery date. For this, patients are seen by both the doctor who will perform the procedure and the anesthesiologist, and necessary blood tests are performed.
During the surgery, spinal (below the waist) anesthesia will usually be applied, but the patient must still come hungry on the day of the surgery. Since fasting periods vary depending on the patient’s age, they are different for each patient and the anesthesiologist’s advice should be followed.
On the day of surgery, the patient comes to the hospital fasting for the specified time. After the procedure is completed, the patient is observed for a while. After the effect of the anesthetic drugs wears off, the patient is fed orally, then a doctor’s check is made and if the patient’s condition is good, he is discharged.
