Izmir Renal Outlet Stenosis Surgery (Pyeloplasty)

Ureteropelvic stenosis is a disorder that occurs when urine from the kidneys cannot reach the bladder. Urine cannot pass through the canal and reach the bladder due to stenosis or obstruction at the junction of the renal collecting system and the ureter.

Urine passes through a tube called a ureter to reach the bladder and can accumulate in the bladder until it is expelled. Some problems may occur in the body due to problems in urine reaching the bladder from the kidneys.

The most important of these problems is renal outflow stenosis. This surgery can be performed on patients using open, laparoscopic or robot-assisted laparoscopic methods. A method such as entering through the urinary tract only with endoscopy and cutting the narrow part from the inside can also be applied to some patients.

What are the Types of Renal Outflow Stenosis Surgery?

Renal outlet stenosis surgery, also known as pyleloplasty, can be performed in two ways: open and closed.

Open pyeloplasty surgery, like other kidney surgeries, is performed using surgical incisions made from the lower part of the ribs, that is, from the right or left side, depending on the location of the kidney in the abdomen.

It requires a larger incision than laparoscopic and robotic techniques, which can be described as closed techniques. The surgery takes approximately 2 to 3 hours. The duration of hospital stay after surgery depends on the doctor’s opinion and the removal time of the placed drainage pipes.

Laparoscopic or robotic pyeloplasty surgeries also give good results.

Everything that is done surgically is the same as open pyeloplasty in operations performed with closed methods, but in this application, a large surgical incision is not made, only 3 or 4 holes of around one centimeter are opened and the surgery is performed by entering the body through the hole with a camera.

Pyeloplasty surgeries performed with this less risky but challenging technique are called “Laparoscopic or Robotic Pyeloplasty”.

It is similar to open surgery in terms of duration. Approximate surgery time is 2-3 hours and the chance of success is quite high. There are no statistically significant differences between closed and open surgical techniques in terms of success. However, there are some advantages in closed surgery compared to open surgery.

These are mainly;

A larger and clearer looking surgery environment with cameras,
There is no large incision scar, that is, it does not leave a scar after the surgery,
It can be counted as being less painful and healing faster in a shorter time.
What are the Diagnostic Methods Used in Renal Outflow Stenosis?

The size and shape of the kidney can be seen, and any blockages or abnormal tissue can be found using imaging tests such as ultrasound, CT scan, or MRI scan. A special test, a voiding cystourethrogram (VCUG), may also be used to evaluate how well urine moves from the kidney to the bladder and out of the body.

Urine tests can help measure urine output and creatinine levels, which can be used to evaluate kidney function. Examples include urine flow rate and urine creatinine tests.

Blood tests measuring blood urea nitrogen (BUN) and serum creatinine can be used to evaluate kidney function and look for any signs of kidney failure.

Renal outlet stenosis is a disorder that occurs when urine from the kidney cannot reach the bladder. Urine does not reach the bladder through the canal caused by this stenosis, which is seen at the junction of the renal collecting system and the ureter. When urine cannot reach the bladder from the kidneys, some problems may occur.

Renal outlet stenosis surgery can be performed on patients using laparoscopic or robot-assisted laparoscopic methods. A method such as entering through the urinary tract only with endoscopy and cutting the narrow part from the inside can also be applied to some patients. After the surgery, you will stay in the hospital for 1 or 2 days.

How long does pyeloplasty surgery take?

The ureter is a pair of tubes, one on the right and one on the left, that carry urine from the kidneys to the bladder, with an average length of 26-28 cm in adults. The place where the kidneys and ureters meet is called the ureteropelvic junction. Sometimes, stenosis may occur in this area and urine cannot be transmitted from the kidneys to the ureter easily, causing swelling in the kidneys (hydronephrosis) and loss of function over time. This is known as ureteropelvic junction (UPJ) stenosis.

It may be congenital or acquired later due to previous surgeries and infections. Congenital causes may include the presence of an extra vein that goes to the kidney and narrows the ureter, but sometimes such a cause of obstruction cannot be found and the inability to transmit urine to the ureter due to the inability of that area to contract well is blamed. It may cause symptoms such as pain and vomiting, which increase especially with fluid intake. It can also cause infections and kidney stone formation due to urine accumulation.

In pyeloplasty surgery, the area with stenosis is removed and the areas thought to be healthy are re-oralized and urine is easily transmitted to the ureter.

Pyeloplasty surgery takes between 2-3 hours. The surgery can be performed openly or more often with closed systems such as laparoscopic or robotic. A stent is inserted between the kidney and bladder and removed after about a month. Hospital stay varies between 1-2 days.
